The effect of complicated cataract surgery on iris structure

Purpose:
To evaluate the changes of iris structure following complicated cataract surgery
Setting:
Anterior Segment Disease Department of Beyoglu Eye Research and Education Hospital
Methods:
Retrospective, single-centre case series involving 12 aphakic eyes of 12 consecutive patients following cataract surgery with anterior vitrectomy in a single centre in Turkey between January 2012 and September 2013.The fellow eyes of patients was considered as a control group. Patients with preoperative ocular disease except for cataract and fellow eye with any surgery history were excluded from study. The distance between iris and scleral spur of study eyes were assessed with anterior segment optic coherence tomography (OCT-Visante,Carl Zeiss Meditec®) from 750µm (IT750) and 2000µm (IT2000).The maximum iris thickness(ITM) and iris area (I-Area) were also evaluated.
Results:
The measurements were taken after 6 months following cataract surgery with anterior vitrectomy .The IT750, IT2000, ITM values was statistically significant low in complicated cataract surgery subjected eyes with respect to fellow eyes.( p=0,02; p<0,01; p=0,03;p<0,01 respectively)
Conclusions:
Secondary intraocular lens implantation could be carried out either iris-claw anterior chamber intraocular lenses(ACIOL) or scleral fixation of a three-piece posterior chamber intraocular lenses(PCIOL) for the correction of aphakia in eyes without capsular support. Especially, the reduction of iris thickness and the changes of iris structure following complicated intraocular surgery could have negative effect of intraocular stability of iris claw anterior chamber lens.
