Refractive outcomes of post-myopic LASIK patient lens replacement surgery utilizing topographically-estimated modified Masket IOL power calculations

Abstract Details



Purpose:

To assess refractive outcomes in lens replacement surgery of post-myopic LASIK patients utilizing topographically estimated modified Masket IOL power selection

Setting:

Coastal Vision Private Practice, Orange, CA, USA

Methods:

One hundred consecutive post-myopic LASIK subjects underwent implant with posterior chamber monofocal IOLs. All eyes had best spectacle corrected visual acuity of 20/25 or better post IOL surgery and no other ocular comorbidity. All eyes had topographic data and available three months postoperative manifest refraction following IOL surgery. The diopters of spherical equivalent LASIK treatment were estimated independently by the three authors based on topographic data without consideration of pre-LASIK refractive information. The IOL power calculations were then done using modified Masket formula. Refractive outcomes were then calculated using known IOL power implants and three month postop refraction after lens replacement surgery. All surgeries were performed by Dan B. Tran, M.D. The outcomes were compared among the above three independent estimates of IOL selection and to the actual patient outcomes which had the aid of realtime video streaming intraoperative aberrometry.

Results:

Final results will be available in September 2014. Preliminary results showed over 50% of patients would have achieved within +/-0.5D of intended target correction utilizing topographically estimated modified Masket IOL power calculation. Regression analysis will also be performed to derive an additional correction factor further optimizing refractive outcomes by better estimation of LASIK treatment degree utilizing topography data differential in central versus peripheral corneal curvatures.

Conclusions:

Topographically estimated modified Masket IOL power selection is a simple and effective way to choose IOL power in lens replacement surgery of previously operated myopic LASIK patients. It serves as a good estimate of IOL power range to be made available during surgery that can be further refined by realtime video streaming intraoperative aberrometry aphakic IOL power selection.
