Evaluation of surgical outcomes of toric intraocular lens implantation using anterior segment swept source optical coherent tomography

Abstract Details
Purpose:
The anterior segment optical coherent tomography (AS-OCT) can measure astigmatism of both anterior and posterior corneal surfaces. The aim of this study was to compare surgical outcomes of toric intraocular lens (T-IOL) implantation using swept source AS-OCT and autokeratometry.
Setting:
Ryugasaki Saiseikai Hospital, Ibaraki Japan.
Methods:
Sixty eyes of 48 subjects with cataract undergoing T-IOL implantation were included. We divided the patients into two groups: in the first group, corneal astigmatism was measured with autokeratometry (KRT group, 30 eyes), and in the second group, total corneal astigmatism (anterior + posterior astigmatism) was quantified with AS-OCT (Real power group, 30 eyes). Cylindrical power and axis placement were calculated with an online toric IOL calculator program. All surgeries were performed by a single surgeon with a same method. Visual acuity, refraction, and keratometry were measured preoperatively and at 3 month postoperatively.
Results:
The mean logMAR uncorrected visual acuity, the mean logMAR best-corrected visual acuity, and the postoperative subjective astigmatism were, 0.07±0.13, -0.05±0.06, -0.75D ± 0.45D in the KRT group and 0.05±0.14, -0.04±0.07, -0.34D ± 0.43D in the Real power group, respectively. Subjective astigmatism showed significant difference between the values (p < 0.05). Comparing values between preoperative keratometric index and Real power, 6 eyes had more than 0.5D differences in corneal astigmatism , and steeper meridian angle differences were more than 10 degrees in 22 eyes, 20 degrees or more in 9 eyes and 30 degrees or more in 7 eyes.
Conclusions:
By measuring both the anterior and posterior corneal astigmatism using AS-OCT, surgical outcomes of T-IOL implantation were significantly improved. FINANCIAL INTEREST: NONE
