Femtosecond laser in cataract surgery: personal experience after the first 100 cases

Abstract Details
Purpose:
To present the advantages of femtosecond laser LensX platform by Alcon in cataract surgery, respectively the possibility of customizing parameters, both preoperatively and intraoperatively, once the suction is applied.
Setting:
Laser Optisan Eye - Clinic, Cluj-Napoca, Romania
Methods:
We made a prospective clinical study over the first 100 cataracts operated in Laser Optisan Eye-Clinic by femtosecond laser assisted technique. The interventions were performed between the 1st of February - 1st of December 2013. Cataracts were classified on a grading scale between 1 to 3. Visual acuity before surgery ranged between 20/320 Snellen (1.2 logMAR) and 20/40 (0.3 logMAR). Patients aged between 37 and 91 years-old.
Femtosecond laser assisted cataract surgery has the following steps: programming of the surgical parameters, docking, centering of programmed parameters according to the limbus and the pupil, correlation of these parameters with the live pictures taken by the incorporated OCT, validation of the parameters and activation of the laser.
The surgical parameters are: capsulorhexis dimension, place and size of the incisions and fragmentation pattern of the lens nucleus.
Results:
In all cases we obtained a round, well-centered capsulorhexis. Dimensions were the same as the pre established ones: 4.5 to 5.2 mm and were chosen according to the pupil's dimension after dilatation. The main incision was 2.3 mm, placed at the limbus and had a zig-zag pattern, that allowed self-sealing in 93 cases (93%). We made two side-ports of 1.1-1.7 mm and had difficulties in placing them at the limbus in 24% of the cases. The nucleus was fragmented as planned in all cases according to the three patterns offered by our platform. No posterior capsule rupture was noticed during fragmentation.
Conclusions:
Phacoemulsification still is the most popular intervention in cataract surgery but remains highly dependent on surgeon's skill and experience. Laser assisted cataract surgery has the tendency to standardize the method and reduce complications related to surgeon's personal skills. FINANCIAL INTEREST: NONE
