Incidence of lens touch during pars plana vitrectomy and outcomes from subsequent cataract surgery

Purpose:

To evaluate cataract surgery complication following lens touch during pars plan vitrectomy

Setting:

St thomas' Hospital, London

Methods:

1399 patients who underwent pars plana vitrectomy in three centers during the period 2001 to 2013. Data of the PPV and lens touch complication were reviewed from an electronic database (Vitreor). Subsequent cataract surgery notes and intraoperative complications were reviewed.

Results:

lens touch incidence was 3.8% (54 out of 1399 patients), patients demographics and presenting complaints were not significantly associated with lens touch, but retinal detachment with PVR, the use of silicon oil and relaxing retinectomy led to more lens touch. Cataract developed in 49 patients and 45 underwent cataract surgery. Nuclear sclerosis developed in 22 patients and 16 (33%) posterior capsular cataract. The duration between PPV and cataract surgery was 1 to 19 months, which is significantly less compared to 1-119 months in control group (p<0.05). In the lens touch group 5 patients (11%) had posterior capsule (pc) rupture and 2 cases had sulcus implants, while the control group had only 2 cases of pc rupture (1.4%). 22 patients had final visual acuity of 0.3 LogMar or better.

Conclusions:

lens touch is an under reported complication. Also, care should be taken when performing the subsequent cataract surgery on eye with lens touch as it carries a significant chance of pc rupture complication. FINANCIAL INTEREST: NONE
