The effect of cataract surgery on anterior segment parameters measured by optical biometry in primary open-angle glaucoma and pseudoexfoliation glaucoma

Abstract Details

Purpose:

Our purpose was to compare the effect of cataract surgery on anterior segment parameters measured by optical biometry in cases with primary open-angle glaucoma (POAG) and pseudoexfoliation glaucoma (PXG).

Setting:

This prospective parallel-group trial was conducted from September 2013 to March 2014 in Ankara Ulucanlar Eye Research Hospital.

Methods:

12 eyes of 12 cases (mean age: 65.7 ± 7.6) with POAG and 14 eyes of 14 age and sex-matched cases (mean age: 68.3 ± 4.3) with PXG were included to our prospective study. All of the cases had uncomplicated phacoemulsification and posterior chamber intraocular lens implantation surgery. In addition to complete ophthalmological examination, central corneal thickness (CCT), anterior chamber depth (ACD) and axial length (AL) measurements of their eyes were measured by optical biometer before the surgery and at the first postoperative month. The pre and postoperative values and the differences between POAG and PXG were compared statistically by paired t test, independent t test and chi-square tests.

Results:

There were no significant changes between the preoperative values of intraocular pressure (IOP) under anti-glaucoma treatment (p=0,92) and AL (p=0.17) between the groups. The mean values of preoperative CCT (p=0.04) and ACD (p=0.01) were significantly smaller in PXG than POAG group. In PXG group, the IOP decreased (p=0.001) but the CCT (p=0.03), AL (p=0.001) and ACD (p=0.004) increased significantly postoperatively. In POAG group, the IOP decreased (p=0.01) but the AL (p=0.001) and ACD (p=0.001) increased significantly postoperatively. But there wasn't a significant difference in pre and postoperative CCT (p=0.2). There were no significant differences between the changes of preoperative and postoperative IOP (p=0.71) and CCT (p=0.49) values between the two groups. But the postoperative increase in ACD was more in PXG (p=0.01) and the postoperative increase in AL was more in POAG (p=0.001).

Conclusions:

Cataract surgery may cause some differences in IOP and anterior segment parameters like ACD, AL and CCT postoperatively in POAG and PXG cases. FINANCIAL INTEREST: NONE
