Our experience with scleral fixated PMMA IOL: a clinical study

Purpose:

to study long term stability and complications of scleral fixated intra-ocular lens (PMMA) implantation following various indications.

Setting:

Several methods are in practice for secondary intra ocular lens implantation into in an aphakic eye. Including scleral-fixated IOL(SFIOL), glued IOL etc. Scleral fixated IOL implantation is one of the oldest procedure amongst them. However, long term stability and complications related with scleral fixated PMMA IOL implantation need to be studied.

Methods:

A retrospective study of 36 eyes (35 patients) between April 2004 to Dec 2013. Best corrected visual acuity, intra-oclular pressure, Slit lamp and fundus examination and B-scan USG and OCT (if required) were done. The indications for SFIOL included aphakia, dislocated/subluxated crystalline lens, dislocated/subluxated IOL, penetrating lens injury with loss of capsular and zonular support. The eyes with pre-existing macular pathologies were excluded from the study.

Results:

36 eyes were followed for an average 7.8 years following SFIOL implantaion. SFIOL was implanted due to aphakia in 6 eyes, trauma in 7 eyes, dislocated/subluxated crystalline lens in 10 eyes and dislocated IOL in 13 eyes. Presenting visual acuity ranged from FC 1mt to 6/36p without correction. Final visual acuity ranged from 6/6 to 6/18. Cylinder corrections required in the operated eye ranged from +1.00 D to -3.00 D(n=36). Post-operative complications included fibrinous uveitis (resolved following treatment) in 1 eye, rhegmatogenous retinal detachment in 2 eyes, secondary glaucoma in 1 eye, RPE atrophy in the macula in 5 eyes (following CME).No eyes had dislocation/subluxation of SFIOL or endophthalmitis during follow up(n=36).

Conclusions:

in this long term study (7.8 years), sulcus fixated PMMA IOL implantation following various indications was found to be safe, effective and economical method for long term vision rehabilitation in 36 eyes of 35 patients. Complications following the procedure were less and relatively high irregular astigmatism was one of the commonest (100%) complications in this series. FINANCIAL INTEREST: NONE
