Influence of OVD use on intraoperative aberrometry

Purpose:

The purpose of the investigation was to determine whether there is a difference in IOL power calculation with intraoperative aberrometry when the anterior chamber is filled with BSS or an OVD during aphakic refraction

Setting:

Private Practice Los Angeles and Eye Surgery Center Beverly Hills, CA

Methods:

Non-randomized concurrent series of 3 groups of 20 eyes each having routine cataract surgery. Following nuclear emulsification, cortical 'clean-up' and posterior capsule polishing the temporal corneal incision was hydrated and intraocular pressure established at tonometrically physiologic levels with BSS. Aberrometry was performed and suggested IOL power recorded. Immediately following the anterior chamber was refilled with an OVD and aberommetry repeated. Two groups had an OVD of pure hyaluronate, each of a different concentration while the third group had a chondrotin-hyaluronate OVD. The measured IOL powers were compared.

Results:

The results of the study indicate that aberommetry calculated IOL power is insignificantly different between BSS and purely hyaluronate based OVDs. However, owing to a difference of Index of Refraction, chondroitin-hyaluronate combined OVDs may induce undercorrection of IOL power by approximately a mean 0.5 D.

Conclusions:

Surgeons who employ intraoperative aberrometry to determine IOL power should be aware that certain OVDs, chondroitin-hyaluronate mixtures in particular, may have influence on calculated lens power.
