Management of dislocated nuclear fragments during phacoemulsification by 20G sutureless vitrectomy

Abstract Details



Purpose:

To describe management of posteriorly dislocated lens nucleus in the vitreous cavity during phacoemulsification by by 20G sutureless vitrectomy.

Setting:

Clinic for Eye Disease 'Prof. dr Ivan Stankovic' University Medical Center 'Zvezdara' Belgrade Serbia

Methods:

Retrospective review of 14 patients with dislocation of nuclear fragment into vitreous cavity during phacoemulsification from August 2012 to January 2014. Sutureless 20G pars plana vitrectomy was performed in all cases.

Results:

Average age of this group is 72 (52-83). Preoperative visual acuity was ranged from hand motion to 2/60. Posterior chamber intraocular lens implantation was made during cataract surgery in 8 eyes. Vitrectomy was preformed immediately after cataract surgery in 6 eyes. Follow-up period was 9,5 (5-19) months. In the group with vitrectomy performed immediately after cataract surgery visual acuity was better than 0,5 in 5 eyes (83%), and less than 0,1 in one eye (17%). In the group with vitrectomy performed after cataract surgery, in the second act, visual acuity was higher than 0,5 in 5 eyes (50%), 0,1-0,5 in 3 eyes (30%) and up to 0,1 in one eye (10%). Secondary glaucoma and uveitis were more often in the case vitrectomy made in the second act and this procedure solve them. Cystoid macular oedema has been noted in one case.

Conclusions:

Nuclear fragments dislocation in the vitreal cavity is serious complication of phacoemulsification. Sutureless 20G pars plana vitrectomy performed in the same act as a cataract surgery because of removing nuclear segments results in better postoperative visual acuity and less complications. Despite of associated pathology like corneal edema and secondary glaucoma, sutureless vitreoretinal surgery show very well anatomical and functional result and shorter postoperative recovery.
