Bimanual continuous curvilinear capsulorhexis for lens subluxation

Abstract Details
Purpose:
Lens subluxation and zonular dialysis are difficult cases for cataract surgery. In these advanced cases, intra- or extra-capsular cataract extraction might be an option. However, large incision could cause severe complication, such as choroidal hemorrhage. Especially, completion of capsulorhexis might be the most decisive factor for a successful surgery.
We performed bimanual continuous curvilinear capsulorhexis (CCC) for cases with phacodonesis due to lens subluxation and assess this technique.

Methods:
Five eyes of 4 patients underwent phacoemulsification and aspiration with bimanual CCC technique, and scleral fixation of intraocular lens combined with pars plana vitrectomy between September and December 2013. bimanual CCC was performed using 2 capsulorhexis forceps inserted through bilateral sideports.
Results:
CCC using bimanual technique was completed in all cases and with capsular hooks or iris retractors, surgical manipulation could be under control. No case showed decreased visual acuity postoperatively, and no complication was observed 3 months after surgery.
Conclusions:
Smaller incision is more ideal for a successful surgery in a case with lens subluxation. To perform micro-incision cataract surgery, CCC should be completed. In a case with zonular dialysis, while it might be difficult to complete CCC with conventional technique, bimanual technique could gain successful results. Bimanual CCC could be eligible for severe cases. FINANCIAL INTEREST: NONE
