Abstract Details
Purpose:
Cataract surgery was performed in a 67-year-old patient. During lens injection, intrastromal intraocular lens was observed: how could it happen and how to solve it.
Setting:
Basurto Universitary Hospital, Basque Country, Spain
Methods:
An experienced surgeon performed a cataract surgery with uneventful phacoemulsification. When the IOL was introduced bevel down through the incision, it was noticed that it was actually being placed intrastromally about 7 mm from limbus. In view of the impossibility of taking it out, it was decided to push it down into the bag breaking the corneal inner layers.
A few days postop a DSAEK was scheduled and performed to replace the Descemet and endothelium.
Results:
After four months follow up, the donor cornea is correctly reattached on the right eye and the best corrected visual acuity is better than preoperatively.
Conclusions:
Iatrogenic corneal dissection as a complication of an IOL injection is an atypical and infrequent complication in cataract surgery. DSAEK surgery was done with successful surgical management FINANCIAL INTEREST: NONE
