Causes of implantable collamer lens (ICL) explantation in a tertiary ophthalmology hospital

Purpose:
Purpose: To analyze the causes of Implantable Collamer Lens (ICL) explantation in a tertiary ophthalmologic hospital.
Setting:
Department of Refractive Surgery Institut CatalĂ de Retina, Barcelona Universitat Internacional de Catalunya (UIC)
Methods:
Methods: Retrospective study in which all patients that had a previous ICL implant surgery at our centre, and that needed an ulterior lens explantation from 2000 until 2017 were studied in order to determine its main causes. 36 eyes from 29 patients were analyzed (47.2% women).
Results:
Mean age was 38.11+-6.45 years old. Mean time between ICL implantation and explantation was 7.35+-4,37 years. Mean ICL power was -14.54+-7.28 diopters. Mean size of explanted ICLs was 12.46+-3.14 mm. Out of the 36 ICLs explanted 61.1% were ICM, 13.88% were VICM0, 8.33% were TICM, 8.33% were VTICM0, 5.5% were VICM5 and 2.7% were ICH. 16 patients were explanted due to the formation of anterior subcapsular cataracts, 7 due to nuclear cataracts, 5 due to posterior subcapsular cataracts, 4 due to intraocular hypertension and 4 due to other causes.
Conclusions:
The main cause for ICL explantation was the appearance of cataracts, most frequently anterior subcapsular. However the main cause for early explantation was the rise of intraocular pressure.
