Comparative study on toric vs monofocal intraocular lenses in patients with cataract and astigmatism

Purpose:
Modern day cataract surgery is a refractive one. Spectacle independence is possible if the astigmatism component is taken care off during cataract surgery. The purpose of this study was to compare aspherical toric intraocular lens (IOL) (study group) implanted bilaterally with bilateral aspherical IOL (control Group) implantation in patients with cataract and corneal astigmatism.
Setting:
Tertiary care hospital in Eastern India
Methods:
A prospective, hospital-based study was undertaken in a tertiary care hospital from April 2016-March 2017.The participants included 84 individuals who had bilateral cataract and bilateral corneal astigmatism of at least 1.25 diopters (D).The patients were randomized to receive either bilateral toric (study group = 40) or bilateral aspheric monofocal (control group = 44) IOL implantation. The main outcomes were assessed at one month and 6 months intervals. The main outcome measures were uncorrected distance visual acuity, residual astigmatism, contrast sensitivity and quality of life.
Results:
Corneal astigmatism was 1.75 (0.95) D and 1.70 (0.84) D in the toric and control groups, respectively. Postoperatively, 33(82.5%) patients in the toric group achieved an uncorrected distance visual acuity of 20/20 compared with 16(36.4%) in the control group (P < .001; odds ratio, 5.23; 95% CI, 2.03-13.48). Spectacle independence for distance vision was achieved in 35 toric patients (87.5%) compared with 14 patients (31.8%) in the control group.Mean refractive astigmatism was -0.65 (0.52) D and -1.75 D (1.00) D. There were no significant changes noted in the two groups based on contrast sensitivity and quality of life
Conclusions:
Unaided distance visual acuity was significantly better in the toric group compared to aspheric monofocal, control group. This resulted in higher rate of spectacle independence for distance vision in bilateral toric IOL group compared with bilateral control group.
