Purpose:
Visual acuity in intermediate distances is degraded after implantations of multifocal intraocular lenses (IOLs), and combination of different near additional powers could improve such binocular intermediate visual acuity. We examined the binocular depth of focus after staged implantation of different additional powers (+4.0, +3.25, +2.75 D).
Setting:
Department of ophthalmology, Tokyo Dental College Suidobashi Hospital, Tokyo, Japan.
Methods:
For 56 patients, multifocal IOL (ZMB00, AMO) with a +4.0 D additional power was implanted in the first eye, and one of 3 near additional powers, +4.0 D (group MM), +3.25 D (ZLM00, group ML), or +2.75 D add (ZKB00, group MK) was implanted in the second eye according to the patient's choice. At 3 month postoperatively, the distance corrected visual acuities (DCVAs, logMAR) at distance, 1.0, 0.5, 0.4, and 0.3 m were examined. Defocus curves with additional spherical refractions of -5.0 D to 2.0 D were also measured under binocular visions.
Results:
There were 21, 18, and 17 patients in the MM, ML, and MK groups. The mean DCVAs were -0.15, -0.17, and -0.18 at distance, and 0.88, 0.07, and 0.06 at 0.3 m, with no significant difference. The DCVAs at 0.5 m of the ML and MK groups were significantly better than those of the MM group (P<0.02, Steel-Dwass multiple comparison). In the defocus curves, there was a significant difference between the MM and MK groups (P<0.001).
Conclusions:
Staged implantations of multifocal IOLs with different near additional powers were effective for obtaining better intermediate vision.
