Comparative analysis of qualitative and quantitative visual outcomes after implantation of a trifocal (AT LISA® tri839MP) and an extended-range-of-vision (Tecnis® Symfony) intraocular lens

Abstract Details

Purpose:

To compare qualitative and quantitative visual outcomes after bilateral refractive lens exchange followed by implantation of two intraocular lenses (IOLs): one extended range of vision IOL, the Tecnis® Symfony, and one trifocal IOL, the AT LISA® tri839MP.

Setting:

Department of Ophthalmology of Centro Hospitalar do Porto, Portugal

Methods:

Retrospective study with 23 patients with bilateral implantation of Tecnis®Symfony IOL (n=20 eyes, group 1) or AT LISA® tri839MP IOL (n= 26 eyes, group 2) after refractive lens exchange. Best-corrected and uncorrected visual acuity (BCVA and UCVA) for distance, intermediate and near and refractive components were evaluated. Quality of vision was evaluated subjectively with a modified Cataract TyPESpecification questionnaire and objectively by an optical quality analysis system - HD Analyzer® - measuring the objective scatter index (OSI), potential visual acuity at fotopic, mesopic and scotopic conditions, modulation transfer function (MTF) and tear film (TF) analysis. Minimum follow-up was 12 months.

Results:

Postoperatively mean UCVA for distance was similar in both groups and of at least 0,10 logMAR (p=0,763). Better near UCVA was achieved with Tecnis®Symfony IOL, however differences were not statistically significant (p=0,334). In all eyes of both groups, postoperative spherical equivalent was within ±1.00D. OSI and MTF values were better in group 1 (p<0,001 for both). The variation in OSI during TF analysis was higher in group 2 (p=0,04). Patients' satisfaction was higher in group 1, none of the patients was dependent of spectacles in both groups and more patients revealed halos during the day in group 2.

Conclusions:

The two IOLs had predictable refractive results and optical performance with all patients achieved spectacle independence for distance, intermediate and near vision. However Tecnis®Symfony IOL provided better quality of vision in qualitative and quantitative analysis. Patient's satisfaction was higher with Tecnis®Symfony IOL.
