A predictability comparison of 4 different IOL power calculation formulas in eyes of long axial length

Abstract Details

Purpose:

To assess the accuracy of 4 different intraocular lens (IOL) power formulas (Barrett Universal II, Haigis, Ladas Super Formula, and SRK/T) using IOLMaster biometry in eyes with long axial length (AXL).

Setting:

1. Department of Ophthalmology, Seoul Metropolitan Government-Seoul National University Boramae Medical Center. 2. Department of Ophthalmology, Jeju National University Hospital

Methods:

A total of 122 eyes with an axial length of >25.2 mm from 122 patients who had uneventful cataract surgeries with implantation of Akreos Adapt AO IOL at a tertiary care center by single experienced surgeon were included. Optical biometer of IOLMaster was used. The predicted refractive outcome using each of 4 different formulas was calculated and compared with postoperative refractive outcome to give the prediction error.

Results:

There was a statistically significant difference in the mean absolute prediction error (MAE) between the 4 different formulas for IOL power calculation (p=0.014) and the Barrett Universal II formula predicted most accurate outcome. The Ladas Super Formula had the second lowest MAE followed by the Haigis formula. The SRK/T had the highest MAE of the 4 methods assessed. The Barrett Universal II formulas yielded the highest percentage of eyes within ±1.0 D and ±0.5 D of the target refraction in this study (91.51% and 73.26%, respectively).

Conclusions:

The Barrett Universal II formulas showed most accurate IOL power prediction in patients with long axial length.
