Enhanced depth-of-field and micromonovision with IOLs vs LASIK vs trifocal IOLs

Abstract Details

Purpose:

To compare the effectiveness, safety profile and tolerance of presyopia correction with an intraocular lens (IOL), either using a monofocal IOL or a trifocal IOL or using a prebyopia correcting depth of field and micro mono vision profile with Lasik.

Setting:

Clearview Institute Refractive Clinic

Methods:

Consecutive patients having undergone presbyopia correcting Lasik, a monofocal IOL implantation using a similare presbyopia correcting approach or a trifocal IOL implantation were followed and then analyzed regrospectively for 2 years. Refractions, topographies, wavefronts, osmolality and any complications were recorded and underwent statistical analysis.

Results:

All approaches yielded statistically significant improvements in uncorrected visual acuity (UCVA) P<.01. The Trifocal group had the best near vision (20/18 P<.1), the Monofcal group the best distance vision (20/15 P<.1) and the Lasik group the best intermediat vision (20/20 P<.3). At year 2 other measures like contrast sensitivity and tear Osmolality were not statistically significantly different. There was a significantly higher rate of retinal tears and detachments in the lens groups (P<.01) and of night vision symptoms in the Trifocal group (P<.01). There were no patients with complete vision loss from any cause.

Conclusions:

All 3 approaches appear to yield acceptable outcomes. The safety profile is potentially better with te LVC approach in particular regarding retinal complications. The ideal approach for any given patient could and should be tailored to his/her particular circumstances.
