Change of eye dominance after pseudophakic conventional monovision

Abstract Details

Purpose:

The purpose of this study is to evaluate whether eye dominance may change after conventional pseudophakic monovision and to elucidate factors affecting the dominance change.

Setting:

Department of Ophthalmology, Konkuk University Medical Center, Seoul, Republic of Korea.

Methods:

A retrospective study was conducted on 70 patients who underwent bilateral conventional monovision cataract surgery. All patients were divided into two groups according to weather dominant eye changed or not. We compared patients’ uncorrected distant visual acuity (UCDVA), uncorrected near visual acuity (UCNVA), best corrected visual acuity (BCVA), spherical equivalent, stereopsis, and the time interval of binocular surgery between two groups.

Results:

The mean age was 71.26 years old, mean interval between binocular surgery was 118 days, and mean postoperative diopter difference was 1.16 diopters. Following bilateral surgery, 22 patients (31.43%) had changed eye dominance, 48 patients (68.57%) had no change. There was improvement in stereopsis, uncorrected distant visual acuity (UCDVA), uncorrected near visual acuity (UCNVA) and best corrected visual acuity (BCVA) in both groups than before surgery. Changed eye dominance group showed better UCNVA in dominant eye (LogMAR 0.19±0.11 vs LogMAR 0.52±0.34) and UCDVA in non-dominant eye (LogMAR 0.07±0.11 vs LogMAR 0.19±0.16) than unchanged eye dominance group.

Conclusions:

There were 22/70 (31.43%) patients with altered eye dominance after conventional monovision cataract surgery. In patients with changed eye dominance, UCNVA in dominant eye and UCDVA in non-dominant eye were superior to those in patients with maintained eye dominance. So, eye dominance has plastic characteristic rather than constant one, and the smaller the difference in visual acuity between the two eyes, the more likely it is that the eye dominance changes after conventional pseudophakic monovision.
