Purpose:
To evaluate clinical outcomes after cataract surgery in a group of patients implanted with a Mix&Match approach using a diffractive multifocal intraocular lens (MIOL) with a lower add power in the distance dominant eye and a diffractive trifocal MIOL in the fellow eye.

Methods:
In a prospective study including 21 patients(43 to 75 years old), who underwent bilateral cataract surgery with a Mix&Match approach combining implantation of a diffractive low-add (+2,75 D) MIOL in one eye, and a trifocal IOL in the fellow eye. Changes in uncorrected and corrected distance, intermediate (80 cm) and near (40 cm) visual acuities (VA) in logMAR, manifest refraction and binocular defocus curve were evaluated during a 12-months follow-up. We also used the Halo&Glare simulator (Carl Zeiss Meditec, Germany) and 2 questionnaires to assess the occurrence and strength of dysphotopic phenomena, patient’s satisfaction and spectacle independence.
Results:
Mean 3-months postoperative spherical equivalent was -0,06 +/- 0,57D, and 97,61% of patients achieved +/-1,0D. The binocular VA were 0+/-0,42logMAR (CDVA), 0,19+/-0,11logMAR (UIVA 80cm), 0,16+/-0,10logMAR (DCIVA 80cm), 0,11+/-0,09logMAR (UNVA 40cm), 0,08+/-0,09logMAR (DCNVA 40cm). The binocular defocus curve showed a VA of 0,2logMAR or better between +0,5 and -3,0 D. The Halo&Glare simulator results using the strength formula found a mild mean strength value of 44,03%. Patients reported never wearing spectacles for distance, intermediate and near distances activities in 92%, 92% and 75% respectively, with occasional perception of halo and glare, 50% describing them as not at all severe.
Conclusions:
We evaluated a new Mix and Match approach in an attempt to offer patients an enhanced near and intermediate vision without compromise in distance vision. It seems to provide an effective restoration of visual acuity for all distances, with high level of visual quality and patient satisfaction.
