Comparison of visual outcomes and objective visual quality after implantation of extended-range-of-vision and trifocal intraocular lenses

Abstract Details

Purpose:

To compare visual outcomes and visual quality as measured by objective scatter index (OSI) after refractive lens exchange followed by bilateral implantation of two intraocular lenses (IOLs): one extended range of vision IOL, the Tecnis® Symfony, and one trifocal IOL, the AcrySof® IQ PanOptix™.

Setting:

Centro Hospitalar do Porto, Porto, Portugal.

Methods:

Prospective comparative study wich included 30 eyes (15 patients). Refractive lens exchange followed by bilateral implantation of Tecnis® Symfony IOL (group 1) or AcrySof® IQ PanOptix™ (group 2) was performed. Patients were evaluated preoperatively, 1, 3 and 6 months postoperative for contrast sensitivity in mesopic and photopic conditions and glare sensitivity by Metrovision-MonPack3®. Best corrected and uncorrected visual acuity (BCVA and UCVA, respectively) for distance, intermediate and near were evaluated. At the last follow-up visit OSI and tear film analysis were measured by an optical quality analysis system using double-pass wavefront method (HD Analyzer®).

Results:

There were no preoperative differences between groups in all the parameters evaluated. Better UCVA for distance, intermediate and near vision were achieved in both groups (p<0.05). Better intermediate and near visual outcomes were achieved with AcrySof® IQ PanOptix (p<0.05). All eyes had a postoperative spherical equivalent within ±1.00D and a binocular UCVA for distance of at least 20/25 (Snellen Scale). There was no significant variation and differences between IOLs in postoperative glare and contrast sensitivity, either in photopic and scotopic conditions. OSI and tear film evaluation were found to be statistically significantly better for Tecnis® Symfony (p<0,01).

Conclusions:

Refractive lens exchange with Tecnis® Symfony or AcrySof® IQ PanOptix™ intraocular lens implantation provided similar and good visual function and contrast sensitivity under photopic and mesopic conditions with low perception of halos by patients. OSI was significantly better for Tecnis® Symfony, however the tear film analysis was also better in this group. As light scatter may be caused by a poor quality or rapidly breaking tear film, the role of the IOL in these results remain uncertain.
