Phakic lens exchange due to glaucoma development

Abstract Details

Purpose:

To describe the case of a patient who underwent refractive surgery with an Implantable Collamer Lens (ICL) on his left eye. Posteriorly, this patient developed glaucoma on that eye.

Setting:

Consorci Sanitari de Terrassa , Barcelona

Methods:

We present a 48-year-old man from China who arrives to our Glaucoma department. His ophthalmology records were radial queratothomies in both eyes, phakic intraocular lens on his left eye and he had been treated with antiglaucomatous drugs on both eyes for 8 months.

Results:

The intraocular pressure curve revealed fluctuant peaks during the day. The optical coherence tomography (OCT) of the optic nerve showed a decreased on the number of the retinal nerve fiber layer (RNFL) on both eyes, which was higher on his left eye. The campimetry revealed a visual field constriction on the left eye, so a IOL exange was performed for a monodical IOL

Conclusions:

Phakic intraocular lens implantation in a patient with glaucoma history or risk factors could have promoted the progression of this neuropathy in different ways: iridocorneal angle reduction, pigmentation changes of the angle, excessive vault or abscense of permeable iridotomies. It is not yer clear which is the most suitable refractive technique in patients with glaucoma. Recent studies lean towards PRK or LASEK. In our case, the earlier radial queratothomies would desestimate those refractive corneal procedures.
