Preoperative intravitreal bevacizumab for diabetic macular edema

Abstract Details
Purpose:
A study of efficacy of intravitreal bevacizumab(IVB) in DME given 1 week prior to phacoemulsification and foldable intraocular implantation surgery(PKE+FIOL).
Setting:
In eyes with coexisting cataract and DME, following cataract operation diabetic macular edema(DME) may exacerbate leading to unsatisfactory post-operative vision. Anti-VEGF agents are being used prior to cataract operation to reduce post-operative exacerbation of macular edema. Study was done in Guwahati Eye Institute.
Methods:
126 eyes of 126 patients were studied between January, 2012 to December,2017. Male =70 ,female= 56. Pre-injection, pre-operative and post-operative anterior and posterior segment examinations,visual acuity,IOT,OCT(CMT) were routine. FFA was done as necessary. Inclusion criteria- cataract with center involving DME (CMT˃300 µ) . Exclusion criteria- uncontrolled diabetes mallitus, diabetic nephropathy, proliferative diabetic retinopathy, foveal hard exudates, prior laser treatment, intraoperative complications. 1 week prior to PKE+FIOL, 0.03 ml of IVB was given into the eye. Follow up- 1 week after IVB and 1 day,15 days and 6 weeks after PKE+FIOL. Patients were asked to monitor blood glucose level weekly.
Results:
total 126 eyes of 126 patients were studied. Mean pre-injection Snellen’s VA was 6/18p(6/60-6/9), pre-operative VA was 6/18(6/60-6/9), VA after 6 weeks was 6/12(6/24-6/9). Mean IOT pre IVB- 18 , Pre-operative -20, at 6 weeks 18 mm Hg. Mean CMT- pre injection 440µ(610 µ-300 µ),pre-operative 400µ(520 µ-280 µ),at 6 weeks 285µ(360 µ-250 µ).No post IVB or post -operative complications.
Conclusions:
IVB prior to PKE+FIOL prevented worsening of DME in 126 eyes (100%) at 6 weeks follow up. Mean CMT decreased from 440 µ to 285 µ and IVB did not cause any intra or post-operative complications. Mean VA after 6 weeks had improved in study eyes to 6/12.
