Vision improvement and satisfaction of patients after advanced monovision refractive surgery

Abstract Details

Purpose:

To analyze correlated satisfaction and vision improvement in patients underwent advanced monovision refractive surgery in a 4 years follow-up.

Setting:

Department of Refractive Surgery - Hospital Oftalmológico de Anápolis, Anápolis, Goiás, Brazil.

Methods:

A cross-sectional observational study of a series of cases was carried out based on review of medical records of participants and 48 months postoperatively in 38 eyes (19 patients). Patients with presbiopic eyes underwent advanced mono-refractive surgery with WaveLight® Allegretto Wave® Eye-Q Laser. At the clinical follow-up, patients satisfaction were evaluated and correlated with their final uncorrected distance visual acuity (UDVA) using Snellen’s scale and uncorrected near vision acuity (UNVA) using Jaeger Eye Chart (“J” scale value) by hypothesis testing and variance analysis.

Results:

Mean of patients age was 49.4 ± 5.5 years. There was a significant improvement in UDVA and UNVA before and after the surgery(p<0.05). When questioned about satisfaction, mean score assigned was 4.29 ± 0.6 on a scale of 0 to 5. Rate of 52.6% of patients were completely satisfied with surgery, although 5.3% that scored a UDVA-UNVA of 1.0-J1 were not completely satisfied. 36.8% patients with UDVA-UNVA of 0.79/J1 or 1.0-J2 and worse were completely satisfied. None patient scored being completely dissatisfied. Variance study not showed significance when analyzing witch improvement, UDVA or UNVA, contributed more to higher satisfaction levels.

Conclusions:

Advanced monovision is a method of presbyopic correction in which one eye is corrected for distance vision and the other eye for near vision. The satisfaction index in the sample studied was high, although less than 50% of patients reached and UDVA-UNVA of 1.0-J1. The level of satisfaction was not statistically correlated with a UDVA-UNVA 1.0J1, but instead with a significant vision improvement comparing to baseline UDVA-UDNA examination and diminished need using glasses most part of time, even after 4 years postoperatively.
