Treatment of difficult lenticular extraction during learning curve of small incision lenticule extraction

Abstract Details

Purpose:

To described management of difficult lenticular extraction cases observed during the learning curve of small incision lenticule extraction(SMILE)

Setting:

It was prospective study carried out at Zulekha Hospital UAE

Methods:

It was prospective study involving the initial 100 cases with myopia (-2 to to 10.00 diopters).undergoing the small incision lenticular extraction at one setup by one doctor. Visual outcomes seen during difficult lenticule extraction cases were studied

Results:

Difficult lenticular seperation was seen in around 7 cases out of initial 50 cases. This lead to two cases of lenticular tear which was treated by removing the lenticular in two pieces.There were 2 cases of side cut tears and remaining 3 cases took more time in seperation of lenticule. There was only 1 difficult cases noted in next 50 eyes. Visual outcome in all patients were same in all the patients

Conclusions:

There is initial learning curve for SMILE which subsides with time. Lenticule dissection and lenticular seperation are important steps and can lead to decrease visual outcomes after SMILE. Most difficult cases were seen in first 50 eyes. But post operative visual outcomes were good in all the cases
