The tomographic and topometric properties of subclinical keratoconus eyes with different clinical presentation of progression

Abstract Details
Purpose:
To analyze the topometric and tomographic properties, including thickness profile data, data from enhanced elevation maps and keratoconus indices of subclinical keratoconic eyes (SbKC) with different clinical presentation of progression.
Setting:
National Ophthalmology Center named after academician Zarifa Aliyeva, Azerbaijan
Methods:
This is retrospective study included bilateral data from preclinical stage of 24 progressive keratoconus eyes (BilatralSbKC group ), 32 subclinical fellow eyes of patients with advanced keratoconus in another eyes ( UnilateralSbKC group) and 35 normal eyes (control group). All eyes in BilateralSbKC group had suspicious tomography and topography finding and one to three years follow up period showed keratoconus progression in one or both eyes. Topographic and tomographic data were measured in all study eyes using the Scheimpflug imaging system.The Mann-Whitney test and Receiver operating characteristic curve analysis was used to compare the data between the groups.
Results:
The mean age was 21.19±2.97, 21.5 ±3.13, and 21.5±2.95 respectively in patients with bilatralSbKC , unilateralSbKC and normal groups.
Comparing bilateral SbKC eyes from unilateralSbKCeyes corneal thickness parameters, the corneal thickness spatial profile and corneal volume values showed lower distribution in bilateralSbKC groups than in unilateralSbKC groups (p<0.001). However other diagnostic variables showed no significant differences between two groups. While comparing all subclinical eyes from normal eyes, all diagnostic parameters showed significant differences, except mean and maximum keratometry and astigmatism. These parameters had high predictive accuracy in bilatralSbKC group than unilateralKC group when compared to normal eyes.
Conclusions:
Eyes in bilateralSbKCgroup showed more severe cornea tissue alteration and more severe progression than fellow eyes of the clinical keratoconus
