Applications of anterior segment optical coherence tomography in the management of corneal abscess by Pseudomonas aeruginosa

Abstract Details

Purpose:

To present the application of anterior segment optical coherence tomography (AS-OCT) in the management of a patient with corneal abscess by Pseudomonas aeruginosa

Setting:

Hospital de la Esperanza - Parc de Salut Mar

Methods:

Report of a case of a patient with corneal abscess by P. aeruginosa and the use of AS-OCT in its therapeutic management.

Results:

Patient presents epithelial defect with a large corneal abscess and corneal melting. It is decided to perform an AS-OCT to assess corneal stromal thickness and the risk of perforation. In the SA-OCT great corneal melting is observed that respects the deep corneal stroma. After resolution of the abscess and closure of the epithelial defect, an AS-OCT is performed to assess the best therapeutic attitude: deep anterior lamellar keratoplasty (DALK) vs penetrating keratoplasty. Due to the observation of a large central leukoma that affects the deep stroma, it is decided to perform penetrating keratoplasty.

Conclusions:

The anterior segment OCT (AS-OCT) is a useful tool for the management of various corneal disorders since it allows an accurate visualization of the anterior segment structures. The case we present, the AS-OCT, allows us to assess the risk of perforation as well as the appropriate therapeutic attitude to be performed.
