Supracilliary microstent implantation with cataract surgery in patients with open-angle glaucoma: five year experience

Abstract Details

Purpose:

Assessment of reduction of IOP and number of medication after implantation of Microstent CyPass (Alcon, USA) in patients with open angle glaucoma. The implantation was performed combined with cataract operation or as a stand-alone procedure. Additionally, the safety of the procedure was assessed.

Eye clinic Cham,Germany

Methods:

In a prospective clinical study, a total of 24 eyes were included. The microstent was implanted following cataract surgery in 24 eyes. Included were patients with a diagnosis of open angle glaucoma ,Shaffer grade 3 or 4, BCVA >0.3, and or medicated IOP between 21-31 mmHg) or those receiving 2-3 IOP-lowering medications. The microstent was implanted ab interno through the perilimbal temporal incision of 1,8 – 2,8 mm under the visualisation with gonio lens in the supracilliary suprachorioidaly space.

Results:

The mean preoperative pressure IOP was 22,3 and the average medications 1,8. After 12 months, the mean IOP in the combined group was 14,9 and after 60 months, 16,5. The average number of IOP lowering medications was 0,7 after 12 month and 1,0 after 60 months. In 2 patients, adherence of the iris to CyPass was observed and then solved byYAG laser treatment. In one patient, the IOP rose due to obstruction. A second CyPass was implanted and pressure dropped to 12mm Hg.

Conclusions:

The implantation of microstent CyPass combined with Phako or a stand-alone procedure is a safe micro invasive procedure for OAG as well as an effective one, significantly lowering IOP and IOP-lowering medication use at 5 years.
