Predictors of adverse outcome in advanced glaucoma

Abstract Details
Purpose:
To find out the predictors of glaucoma related blindness in advanced glaucoma.
Setting:
Advanced Eye Centre, PGIMER, Chandigarh, India
Methods:
It was a crosssectional observational study conducted at tertiary care centre, recruiting patients >30 years of age who were diagnosed as primary/secondary glaucoma presenting with VA < 20/200 or VF showing less than 200 isoptre in at least one eye due to glaucoma with a minimum follow-up of 1year. Patients were classified into two groups i.e, advanced and end stage glaucoma (Vision: light perception and no light perception or visual fields with a MD> -12.00 dB in the worse eye). The outcome was assessed according to visual acuity and visual fields to see predictors of blindness.
Results:
Total 169 patients, 99 primary/ 70 secondary, mean age 62.67/ 54.65 years respectively. Trauma- 21.4%, complicated cataract- 15.7% and silicon oil tamponade -14.3% respectively were seen in secondary glaucoma. Significant delayed diagnosis in primary glaucoma versus secondary glaucoma (57.57% vs 32.85%) was seen. Non compliance was significantly higher in primary glaucoma 34.3% vs 17.1%, In the univariate analysis, literacy, income status , locality , delayed diagnosis of glaucoma and missed diagnosis were statistically significant by visual acuity criteria. 18/33 (54.5%) patients in GFS and 105/136 (77.2%) with no surgery reached end stage glaucoma as per Visual fields outcome criteria (p=0.011)
Conclusions:
Lack of awareness, delay in diagnosis, low literacy and income were associated with poor visual outcome. Patients who underwent glaucoma filtration surgery had significantly better visual outcome in comparison to patients managed medically.
