Purpose:
To report a case of a patient who developed contemporary bilateral acute primary angle-closure glaucoma.
Setting:
Ophthalmic Unit - “ Dolo Hospital” - Dolo (Venice) – Italy
Methods:
A 52-year-old Caucasian woman presented acute primary angle-closure glaucoma in both eyes with increase in intraocular pressure (IOP) that results in dramatic symptoms, causing patients to promptly seek medical attention.
Results:
Maximal medical therapy was immediately initiated followed by laser iridotomies in both eyes but the IOP always remained above 30 mmHg . Facoemulsification with core vitrectomy were performed in both eyes with normalization of IOP .
Conclusions:
Bilateral acute angle-closure is a rare complication in patients with primary angle closure suspects . The clear lens extraction is the only surgical solution when the raised IOP is lessresponsive to medical and laser therapy.
The prophylactic laser iridotomy in eyes with potentially occludable angles is a simple and safe office procedure that can prevent potentially devastating visual outcomes.
