Femtosecond laser-assisted cataract surgery (FLACS) in peculiar scenarios

Abstract Details
Purpose:
To explore the limits of FLACS in various peculiar clinical scenarios.
Setting:
Aravind Eye Hospital, Pondicherry
Methods:
We used FLACS for cataract surgery in the following peculiar scenarios :
a patient with Anterior Lenticonus,
a patient with Implantable Collamer Lens(ICL) for lens removal and cataract surgery,
a traumatic cataract,
a white intumescent cataract,
a dense cataract.
Scanning Electron Microscopy was done for the anterior lenticonus capsule and the white intumescent capsule.
the femtosecond laser was followed with the phacoemulsofocation and all post-op visual acuity was UCVA of 6/6
Results:
We found that FLACS was safe and possible in these tricky scenarios but there are a few pitfalls.
Uncut nucleotomy of the lens chop pattern was present in the ICL patient.
Scanning Electron Microscopy of White Cataract capsule showed uncut areas.
the dense cataract removed had a less phaco time, with new technique - doughnut chop to remove the fragmented nucleus with phaco
Conclusions:
FLACS is a promising technology for peculiar scenarios in cataract surgery in addition regular cataract.
