Cytokines, chemokines, and flare in anterior chamber after femtosecond laser-assisted cataract surgery

Abstract Details
Purpose:
To analyze anterior chamber inflammation (cytokines, chemokines and flare) after pretreatment by a femtosecond laser platform during cataract surgery, in comparison with the manual procedure.
Setting:
Eye Clinic, Department of surgery and translational medicine, University of Florence, Italy
Methods:
Aqueous humor was collected from 20 eyes after femtosecond laser pretreatment and from 20 eyes at the beginning of routine cataract surgery. The levels of 14 cytokines and chemokines were measured by a multiplex array system. Surgical parameters (suction time, laser time, effective phaco-time, EPT) were recorded. Flare in the anterior chamber was measured by laser photometry preoperatively and at 1 day, 7 days after surgery.
Results:
The EPT was significantly lower in femto group compared to controls. In the femto group the concentrations of IL-6, IL-8, IL-10, IL-12, VEGF and IFN-γ were significantly higher than control group. The flare in anterior chamber measured with flaremeter was not significantly different between groups at all time points. No correlation was found between cytokine concentrations and age or cataract density in both groups and between cytokine levels and suction time or laser time in the femto group. No correlation was also found between postoperative aqueous flare and EPT in both groups.
Conclusions:
Despite the rise of proinflammatory cytokines in the aqueous humor after femtosecond laser pretreatment, the anterior chamber flare after cataract surgery was similar to controls. This might be due to the lower effective phaco time needed after pretreatment. Further studies are needed to confirm these findings.
