Femtosecond laser-assisted cataract surgery after penetrating keratoplasty

Abstract Details

Purpose:

To report the outcomes of 3 eyes of 3 patients that underwent femtosecond laser-assisted cataract surgery (FLACS) and toric intraocular lens implantation (tIOL) with arcuate keratotomy (AK) for cataract formation and high post-keratoplasty astigmatism.

Setting:

Interventional case series in Bayındır Kavaklıdere Hospital

Methods:

Patients had history of penetrating keratoplasty (PK) for keratoconus, corneal dystrophy or corneal scar secondary to herpetic keratitis. The cataract surgeries were performed using LenSx laser system (Alcon, FW, US) and Centrion (Alcon, FW, US) devices in all eyes. All eyes had tIOL implantation (2 of had T9 and 1 had T8) and AK with femtosecond laser system. Manifest refractive astigmatism (RA), spherical equivalent (SE), corrected visual acuity (CVA) at postoperative month-6 were recorded

Results:

The mean preoperative CVA significantly improved from 0.86±0.40logMAR (range: 1.30-0.52, logMAR) to 0.00 logMAR at postoperative 6th month. The preoperative RA was 6.41±3.37 Diopter (D) (range: 3.00-9.75 D), and significantly decreased to 3.66±1.46D (range: (-2.00)-(-4.75) D). The SE improved from -6.08±3.87 D [range: (-3.25)-(-10.5) D] to -1.41±2.78D [range: (1.75)-(-3.50) D].

Conclusions:

FLACS and tIOL implantation, which can be combined with AK, seem to be safe and effective procedure in patients having cataract formation and preexisting high astigmatism secondary to PK.
