Minimising refractive lenticule extraction complications: initial Canadian experience

Purpose:
We sought to determine the complications in an initial series of patients undergoing Small Lenticule Extraction (SMILE) and identify the factors that minimized them.

Clearview Institute Refractive Clinic
Methods:
The first consecutive 500 eyes were retrospectively analysed for accuracies of refractive outcomes, retreatment rate and occurrence of complications. All patients underwent pre and post operative cycloplegic refractions and wavefront as well as topographic scanning. They were monitored for up to 1 year.
Results:
There was no loss of best corrected visual acuity. 9/500 eyes required a retreatment which resulted in achieving the desired target. 4/9 retreatments were via customized Crosslinking and 2/9 underwent Lasik, 3/9 were corrected with PRK. 5/500 eyes showed a post-operative intraocular pressure (IOP) spike that resolved when treated without sequelae. The posteroperative steroid treatment was reduced and no further cases occurred. Astigmatism correction was optimized wiht manual adjustment of the interface lens. There were no cases of keratoectasia or infection.
Conclusions:
Complications form the SMILE procedure appear to be rare and generally of no long term consequence. A reduction in the postoperative steroid treatment is warranted.
