Clinical prevalence of familiar association in keratoconus (KCN): tomographic evaluation of KCN patient parents

Purpose:
To try and identify suspicion or clinical keratoconus in known KCN families.
Setting:
LaserVision.gr Clinical and Research Eye Institute, Athens, Greece
Methods:
In a prospective study, 165 keratoconus patients had their parents imaged by Scheimpflug tomography mapping. The tomographies were reviewed by an independent, blinded to the process expert observer for known and also suspicious signs of early ectasia.
Results:
The study included 34 (21%) female and 131 (79%) male known KCN subjects. Mean age was 25 ± 7 years (range, 15-45 years). KCN or suspicion for KCN was identified in one of the two parents evaluated in 90.6% of these cases and in both parents in 8% of the cases.
Conclusions:
KCN inheritance pattern in the Greek population appears to be significantly greater than the percentages known so far. Further genetic testing may confirm or even amplify these data. Clinical relevance dictates close screening for KCN in all families with a KCN member and appropriate caution in laser refractive surgery given to the potential of ectasia.
