Abstract Details
Purpose:
To report and investigate the risk factors and the treatment of peripheral ulcerative keratitis, a new complication occurring post Collagen Cross Linking (CXL)
Setting:
the study was conducted in Beirut Eye And ENT specialist Hospital including keratocunus patients presenting for surgery between January 2010 and December 2016
Methods:
A retrospective study including 474 eyes of 294 patients Their medical files, visual acuities, operative reports and post-operative follow up were reviewed. The CXL with riboflavin Ultraviolet A light were performed by the same surgeon with the assistance of the same nurse. Chi square test (Fisher's Exact Test) and independent t-test were used to compare the variables. A P-value <0.05 was considered statistically significant.
Results:
10 eyes of 7 patients (2.1%) developed a late onset peripheral ulcerative keratitis with or without corneal haze and sterile infiltrates. The complications occurred between 3 months and 6 years post-op. Their mean age of 41 ±6.4 years was higher than the age of the patients with non-complicated surgery 22.25± 8.5 years (p=0.001). Three patients had autoimmune conditions. The sex, the presence of intra stromal ring segments, the keratometry and the thinnest pachymetry were found to be non-significantly different between both groups. All patients responded positively to steroid treatment, only one had a relapse which resolved with cyclospirne drops.
Conclusions:
Long term follow up post CXL is necessary to uncover the late onset peripheral ulcerative keratitis. It is a treatable condition associated older age and autoimmune conditions but has a good visual outcome.
