Abstract Details

Purpose:

To report the six-month clinical and topographic outcomes in patients with keratoconus after accelerated corneal crosslinking (CXL) using an increased total dose of 7.2 J/cm2 with dextran-free isotonic riboflavin solution .

Setting:

Ondokuz Mayis University, Faculty of Medicine, Samsun, TURKEY

Methods:

The accelerated CXL treatments (KXL system) were performed using a 10-minute riboflavin 0.1% (Vibex Rapid) with hydroxylpropyl methylcellulose (HPMC) soak and a 10-minute ultraviolet-A (UVA) irradiance at a level of 12 mW/cm2. This corresponds to a total radiant exposure of 7.2 J/cm2. Preoperative and 1, 3, and 6 months postoperative examinations were performed.

Results:

The study enrolled 25 eyes of 25 patients. The mean uncorrected distance visual acuity showed a statistically significant improvement, from 0.49 ± 0.27 logMAR preoperatively to 0.28±0.30 logMAR 6 months postoperatively (P<.01). The mean keratometry readings also changed significantly. There were no statistically significant changes in the endothelial cell density between preoperatively and postoperatively

Conclusions:

We observed that new riboflavin formulation using an increased total dose of 7.2 J/cm2 seems to be quite efficient to treat and halt the progression of keratoconus without significant endothelial toxicity and may be the standard therapeutic protocol in the future.
