Flattening and progressive thickness reduction after photorefractive keratectomy followed by riboflavin-UVA collagen cross-linking: case report

Abstract Details

Purpose:

To report a challenging case of cornea flattening and progressive thickness reduction after Photorefractive Keratectomy followed by riboflavin-UVA collagen cross-linking in both eyes.

Methods:

Case report

Results:

A 34-year-old female was referred for progressive visual acuity decreasing. In 2014 was submitted to PRK followed by corneal collagen cross-linking after 4 weeks. Refraction: +3.00D -1.00D 70° 20/150 and +7.00D -1.50D 115° 20/200. The slit lamp exam demonstrated a thinning cornea and haze grade 3+. The previously topography was compatible to asymmetric astigmatism, keratometric of 47.2D and 47.7D, pachymetry of 506µm and 503µm. Corneal tomography (2017) shows Kmax of 34.2D and 29.8D, pachymetry 358µm and 271µm. A scleral lens were fitted to the patient with final visual acuity of 20/20 and 20/25.

Conclusions:

A PRK performed in a patient diagnosed with keratoconus followed by a corneal cross-linking is an unpredictable procedure concerning to the the corneal topography, hazing and progressive thickness reduction. The current case shows a patient presenting a reduction of approximately 13 D and 19 D of the Kmax and 148µm and 232µm of pachymetry in right and left eye respectively.
