Effect of topical prostaglandin analogs and β-blockers on corneal biomechanical properties

Abstract Details

Purpose:

Topical prostaglandin analogs (PGA) have been shown to increase the expression of matrix metalloproteinases in the ciliary body and the cornea and thereby decrease the intraocular pressure and the corneal thickness. These properties of PGA suggest their influence on the corneal biomechanical properties. The purpose of this study was to compare the effect of topical PGA and β-blockers on the corneal biomechanical properties.

Setting:

Inouye Eye Hospital located in Tokyo and Miyata Eye Hospital located in Miyazaki.

Methods:

The subjects were 45 patients with open-angle glaucoma in which PGA (28 patients) or β-blockers (17 patients) had been used for 3 months or more in one eye. The corneal biomechanical properties were measured using Corneal Visualization Scheimpflug Technology (Corvis ST, Oculus Optik-geräte GmbH, Wetzlar, Germany). Paired t-test was used to compare the corneal biomechanical parameters between eyes treated with PGA or β-blocker and their contralateral eyes. Linear regression analysis was used to investigate the effect of PGA on the corneal biomechanical parameters adjusting for intraocular pressure (IOP) and central corneal thickness (CCT).

Results:

CCT in eyes with PGA treatment was significantly thinner than that in contralateral eyes (531.3±35.1µm, 547.4±32.9; p<0.0001). IOP in eyes with PGA treatment was significantly lower than that in contralateral eyes (14.2±2.2 mmHg, 15.5±2.9; p=0.004). Deformation amplitude (DA) of the highest concavity in eyes with PGA treatment was significantly larger than that in contralateral eyes (1.182±0.12 mm, 1.033±0.10; p=0.011). DA in eyes with β-blocker treatment and contralateral eyes was similar. After adjusting for IOP and CCT, there was no significant difference in DA between eyes with PGA treatment and contralateral eyes.

Conclusions:

The results suggest that the corneal biomechanical parameters are altered by topical PGA treatment mainly due to the decrease of IOP and CCT.
