Femtolaser-assisted posterior lamellar keratoplasty with inverted graft formation

Abstract Details

Purpose:

to evaluate the clinical results of posterior lamellar keratoplasty with inverted formation of the translant using a femtosecond laser

Setting:

Ufa Eye Research Institute, Russia

Methods:

The study included 14 patients (14 eyes) with secondary corneal endothelial dystrophy after cataract surgery, men - 4 (28.6%), women - 10 (71.4%). Patients underwent endothelial keratoplasty with an inverted graft formation (110 μm in thickness, 7.5 mm in diameter), using femtosecond laser LDV Z8 (Ziemer, Switzerland). The observation period is 12 months

Results:

Preoperatively a significant decrease of the endothelial cells density (ECD) was detected, accompanied by edema, a violation of transparency. Preop visual acuity (VA) 0.04±0.01, corneal thickness (CT) - 670±134 mc, average ECD - 1080±130 cells/mm2. 6 months after the VA increased to 0.4±0.15, the CT decreased to 580±140 μm, the average ECD was 1540 ± 250 cells/mm2. In 1 year the VA was stable at 0.38±0.2 and the CT was 589±98 μm (р≥0.08), ECD was 1215±180 cell/mm2 (р≤0.05). In 1 case, the failure of the transplant was noted, which required a second transplantation

Conclusions:

The femtolaser-assisted posterior lamellar keratoplasty with an invert graft formation has shown a high clinical efficacy, providing an increase in visual acuity in 10 times, a slight loss of ECD (by 21%) and a high transplant engraftment (92%) during the 1 year follow-up
