Sutureless fresh amniotic membrane transplantation in the management of resistant corneal epithelial defects: an OCT study

Abstract Details

Purpose:

To demonstrate the efficacy of fresh amniotic membrane and the ease of this sutureless technique.

Setting:

Nour El Hayat Hospital, Watany Eye Hospital, Ain Shams University

Methods:

Thirteen eyes of 13 consecutive patients presenting with corneal epithelial defects unresponsive to medical treatment were enrolled. Seven cases were post PRK, 4 were post keratoplasty, a case of each of alkali burn, and another of an amoeboid ulcer. A single layer of freshly cultivated amniotic membrane was fashioned and laid over the cornea, dried by a strong air current, a bandage contact lens was put, and the eye left covered for 24 hours. Outcome measures were healing epithelial defect, corneal morphology, stromal thickness changes at site of the defect as measured by OCT (OptoVue, USA), and vascularization.

Results:

Follow-up extended to 6 months. The defect was closed in all 13 cases, OCT showed that the mean residual stromal thickness at the base of the defect to be 348 (+/- 89 ) microns before surgery. The mean total corneal thickness was 580 (+/- 65) on day 1 post surgery. Progressive reduction was observed and at 6 months the mean thickness was 420 (+/- 72) micron. During the course of follow up no cases developed neovascularization.

Conclusions:

Sutureless fresh amniotic membrane is both effective and easy.
