Penetrating keratoplasty in patients with corneal perforations and fistulas

Purpose:
To study the functional results of penetrating keratoplasty in patients with corneal perforations and fistulas. The ceratoplasty is performed after previous conservative and surgical treatment to stop acute destructive processes in the cornea.
Setting:
Academician S.N. Fedorov St.Petersburg Branch of the IR&TC "Eye microsurgery", St.Petersburg, Russia.
Methods:
All 29 patients had previously underwent a biological coating (closure) of the сorneal perforation, the dimensions of which varied from 0.2 to 6.0 mm. The diameter of the transplants was selected individually and varied from 6.0 to 9.0 mm, often exceeding the diameter of the trepanation bed by 0.5-3.0 mm. The preoperative level of corrected visual acuity varied from 0.01 to 0.09.
Results:
Biological results. After 1 year clear transplant engraftment were observed in 65.2% of cases; partially clear and not clear - 34,8%.
Optical results. In 20.6% the visual acuity is achieved up to 0.05, 0.06-0.09 - 65.6%, 0.1 and higher CDVA established in 13.8% of patients. Hydrodynamic disorders were noted in 2 patients with aphakia and 2 patients with pseudophakia, it required the appointment of antihypertensive drugs and it allowed to achieve the normalization of the ophthalmotonus.
Conclusions:
Penetrating keratoplasty in patients with corneal perforations and fistulas which is performed after preliminary conservative and surgical treatment of patients with relief of acute destructive processes in the cornea could be very effective intervention, allowing to achieve clear transplant engraftment in more than half of the cases and keep perspectives for further rehabilitation of this severe category of patients.
