Visual and refractive outcomes of intrastromal corneal ring segment implantation in paracentral keratoconus with perpendicular topographic astigmatism and comatic axis

Abstract Details

Purpose:

To assess the visual and refractive outcomes of implanting intrastromal corneal ring segments (ICRS) in paracentral keratoconic eyes with perpendicular topographic astigmatisms and comatic axes.

Setting:

Instituto Universitario Fernandez-Vega, Oviedo, Spain

Methods:

56 eyes with paracentral keratoconus with perpendicular topographic astigmatism comatic axes were evaluated before and after the implantation of Ferrara-type ICRS. A 150° arc-length Ferrara-type AFR6 ICRS (AJL Ophthalmic, Spain) was implanted in all cases. Patients were divided into two groups: Group I: The implantation axis of the ICRS was coincident with the flat topographic axis, and Group II: The implantation axis of the ICRS was coincident with the comatic axis. Uncorrected (UDVA) and corrected (CDVA) distance visual acuity, comatic aberration and residual refractive errors were recorded before and 6 month after surgery.

Results:

The mean UDVA rose after surgery in both groups (P< 0.0001). The mean CDVA varied from 0.85 ± 0.14 to 0.90 ± 0.14 (P=0.03) and from 0.79 ± 0.12 to 0.93 ± 0.09 (P=0.003) for the groups I and II, respectively. For the group I, six months after surgery, 18% of eyes lost one line of CDVA compared to preoperative values. For the group II, none of the eyes lost lines of CDVA after surgery. The spherical equivalent declined after surgery in both groups (P < 0.001). The comatic aberration only decreased in the group II (P < 0.001).

Conclusions:

Ferrara-type ICRS implantation in keratoconus that meets the morphological characteristics of the sample under study provides good visual and refractive outcomes, providing better results the implantation axis of the ICRS was coincident with the comatic axis.
