Intrastromal corneal ring segment implant in paracentral keratoconus with coincident and non-coincident topographic and comatic axes

Abstract Details

Purpose:

To evaluate the results of implanting intrastromal corneal ring segments (ICRS) in paracentral keratoconus with coincident and non-coincident topographic and comatic axes.

Setting:

Hospital Clinico San Carlos (Madrid, EspaƱa)

Methods:

A retrospective observational study was conducted in a series of patients with paracentral keratoconus and coincident comatic and topographic axes (group one) and non-coincident axes (group two) patients who underwent implantation of one or two ICRS. The variables analyzed preoperatively and one week, one month and six months after surgery were: visual acuity with and without correction, topographic indexes and corneal aberrations determined by Oculus Pentacam elevation topography, as well as postoperative complications. Statistic analysis with SPSS was performed.

Results:

A total of 221 patients were included (112 in group one and 109 in group two), mean age of 37.43 +/- 11.89 years-old and 34.17 +/- 11.11 years-old, respectively. Visual acuity (two lines improvement). topographic astigmatism (50% reduction), keratometry (flattest axis, the most curved, maximum and average) and high-order corneal aberrations (coma type) improved significantly after surgery in both groups, without statistically significant difference between the groups. There was an infection rate (1.8% and 0.9% respectively), extrusions (11.6% and 10.1%) and paradoxical or insufficient refractive results (2.7% and 2.8%) similar in both groups.

Conclusions:

The results do not show statistical difference in the efficacy of the ICRS implant in both groups of keratoconus, with a similar profile of adverse effects in both groups.
