Super thin DSAEK grafts with thickness 60 microns and less

Abstract Details

Purpose:

To introduce a safe standardized single pass microkeratome technique for preparation in the eye bank without perforation of Super Thin donor grafts with thickness 60 µm and less for DSAEK

Setting:

Lions Eye Institute for Transplant and Research, Tampa, Florida, USA

Methods:

All grafts prepared at the Lions Eye Institute for Transplant and Research, Tampa, Florida, USA. New variables in corneal physiology identified and used to calculate and achieve targeted graft thickness. Standard microkeratome Evolution 3E (Moria, France) was used. Previously developed Single Pass HPAC method for preparation of the Ultra Thin DSAEK Grafts was utilized. Residual bed thickness 60 microns or less, absence of perforation, acceptable Slit lamp, Back light, specular microscopies, OCT were determined as success

Results:

49 Super Thin donor grafts were prepared. No perforations, endothelial damage or other complications occurred. Endothelial cell density before pre-cut - 2825 ± 139 cell/mm²., after - 3031±40 cell/mm². Central corneal thickness in average: before preparation - 468 microns and after preparation – 55 microns (thinnest graft 47 and thickest 60 microns). Cap diameter was an average 9.2mm. All grafts were transplanted. Reported 4 technical difficulties during insertion

Conclusions:

The super thin grafts for DSAEK with thickness 60 microns and less can be safely prepared in the eye bank by standard microkeratome without perforations
