The shifting of main corneal reference points accompanying decreased biomechanical property promotes early finding of keratoconus

Abstract Details

Purpose:

To investigate the characteristic of the shifting of main corneal reference points during the progression of keratoconus.

Setting:

Hankou Aier Eye Hospital

Methods:

46 patients with clinical keratoconus in one eye and/or subclinical keratoconus in the fellow eye were identified. Pentacam HR and Corvis ST parameters were analyzed and combined using different artificial intelligence methods. Grouping criteria are based on the Scheimpflug keratoconus grading standards adapted from the classical Amsler–Krumeich standards.

Results:

The distance between the thinnest-point and pupil-center increases gradually during the progression of keratoconus. The increasing of distance between the corneal-apex and pupil-center initiates from the keratoconic stageⅠ-Ⅱ. Compared with the control group, the thinnest-point in keratoconic pre-stage shifts from the pupil-center more significantly, accompanying with the decrease of the corneal stiffness and CBI, but without the decrease of BSCVA and changes of anterior surface Pentacam-derived topometric indices and corneal thickness progression index. During the progression of keratoconus, the decrease of BSCVA shows significant negative correlation with the distance between the corneal-apex and pupil-center.

Conclusions:

Our findings suggest the shifting of main corneal reference points may be prior to the change of corneal shape, and it could be useful in detecting the patients in keratoconic pre-stage.
