Corneal regenerative therapy in postoperative herpetic keratitis: case report

Abstract Details

Purpose:

To present a case of postoperative herpetic keratitis with severe scaring and stromal thinning treated with corneal ReGeneraTing Agent (Cacicol®, Thea Laboratoires, France).

Methods:

Two weeks after cataract surgery, 75-year-old patient developed herpes labialis and presented with redness and severe pain in his left eye. HSV keratitis was diagnosed. Topical Ganciclovir 5x daily and Aciclovir 5x 800 mg orally a day were applied without any improvement after 4 month of treatment. Before RGTA® therapy best corrected visual acuity (BCVA) OS was 0.1. Severe scarring of the corneal stroma OS was noted. Topography map showed significant irregularities in the surface of the cornea and stromal thinning (thinnest 381 µm) was found in pachymetry map.

Results:

We applied RGTA® (ReGeneraTing Agents), Cacicol® which is a structural analogue of glucosaminoglycan and heparan sulphate. Throughout the treatment the patient was applied with 3 batches of Cacicol® (1 drop once a week for 5 weeks) and between each batch four week breaks were made. After a series of three batches of Cacicol®, we noted visual improvement of the left eye (BCVA from 0.1 to 0.6) and an improvement in corneal transparency and in parameters in topography and pachymetry map (thinnest 402 µm). In the course of treatment we did not observe any local or systemic side effects.

Conclusions:

Cacicol® accelerates the recovery of cornea, improve the visual acuity and transparency of the cornea and it may be used in post-inflammatory scarring of the cornea after herpetic stromal keratitis. Cacicol® is safe and convenient to use.
