The trabecular micro-bypass iStent: an effective method of treatment for patients with open-angle glaucoma

Abstract Details

Purpose:

The aim of this study is to evaluate safety and effectiveness of combined treatment - phacoemulsification cataract surgery with intraocular lens implantation, and trabecular micro-bypass stent (iStent by Glaukos) implantation into the Schlemm's canal.

Setting:

In 2017, 29 treatments were performed at the Okulus Plus Center of Ophthalmology and Optometry in Bielsko-Biała. The procedures were performed in 19 patients, 14 women and 5 men, aged 59-82 years, diagnosed with cataract and open-angle glaucoma of low or medium degrees.

Methods:

Before the surgery, eye diagnostics were performed and prophylaxis was recommended to reduce the risk of infection and post-operative complications. The procedure was performed using the phacoemulsification method, with a 2.2 or 2.7 mm corneal incision and implantation of a monofocal, toric or multifocal lens, and implantation of iStent into Schlemm's canal in the inferior nasal quadrant. Postoperative complications were assessed and the intraocular pressure (IOP) and the amount of anti-glaucoma medications used before surgery and at 1., 3. and 6. months after surgery were compared.

Results:

The most common side effects were: hyphema, temporary IOP increase in patients using dexamethasone after surgery and foreign body sensation. After 1 months of observation a reduction of IOP was achieved by 4.38 units in comparison to the IOP before surgery. Furthermore, the required number of IOP-lowering medications has been reduced by 1.44 in this time period. After 3. and 6. months of observation a significant reduction of IOP and a reduction of the amount of IOP-lowering medications were also achieved.

Conclusions:

The combined treatment of phacoemulsification with intraocular lens and iStent implantation is a safe procedure. It lowers effectively intraocular pressure and reduces the required amount of anti-glaucoma medications or eliminates them completely.
