Aniridia associated with lens coloboma and glaucoma: a case report

Abstract Details

Purpose:

To describe the management of cataract extraction with aniridia lens implantation and refractory glaucoma in a case with congenital aniridia.

Setting:

Department of Ophthalmology. Hospital General Universitario de Alicante. Alicante. Spain

Methods:

We present the case of a 40-year-old male patient affected by congenital aniridia, bilateral lens coloboma, cataract and glaucoma. The patient had cataract removal with aniridia lens implant in both eyes: in the right eye with sulcus fixation and in the left eye with scleral fixation. The patient underwent Molteno valve implantation for refractory glaucoma following the cataract surgery.

Results:

The aniridia lens which was implanted in the ciliary sulcus remained unstable and the other one with scleral fixation remained stable. The haptics of aniridia lens have been secured to the sclera through scleral pockets using iris hooks retractores. Six month after the surgery, the best-corrected visual acuity was 20/200 and the intraocular pressure was stable at 20 mmHg in both eyes with topical pharmacological management consisting of timolol 0.5% and bimatoprost 0.003%. The photophobia improved significantly after surgery.

Conclusions:

Implantation of an aniridia intraocular lens provided good results in our patient, despite the variety of endogenous problems associated with aniridia. The use of an aniridia lens can reduce photophobia in these patients with functional and esthetic results.
