Comparison of surgically-induced astigmatism (SIA) and corneal wavefront aberration in phacoemulsification between 1.8mm and 2.75mm clear corneal incision

Abstract Details

Purpose:

This prospective randomized clinical study was to compare the differences between two clear corneal incision (CCI) sizes on the surgically induced astigmatism (SIA), Root Mean Square (RMS) and corneal wavefront aberrations.

Setting:

Ophthalmology Clinic, Universiti Kebangsaan Malaysia Medical Centre

Methods:

Phacoemulsification was done on 59 eyes which included 28 eyes having 1.8 mm and 31 eyes having 2.75 mm CCI. ATLAS corneal topography was done preoperatively and 1 month postoperatively. The data was used to calculate SIA, RMS and corneal wavefront aberrations.

Results:

There was no significant differences for central and simulated SIA between 1.8 mm and 2.75 mm group (p = 0.081, p = 0.641 respectively). However, the mean simulated SIA in 2.75 mm (0.914±0.603) was higher than in 1.8 mm group (0.556±0.496). Besides that, there was also no significant changes in total RMS and high order RMS (HoRMS) with p = 0.078 and p = 0.847 respectively. In 1.8 mm group, there were no significant changes in all Zernike aberrations, however, in 2.75 mm group, there were significant changes in WTR/ATR Astigmatism (p = 0.017) and Horizontal Coma (p = 0.035).

Conclusions:

Phacoemulsifications using 1.8mm CCI is superior than 2.75mm CCI in producing significantly less SIA changes and no significant corneal wavefront aberration.
