Systematic preoperative SD-OCT before cataract surgery

Abstract Details
Purpose:
To assess the sensitivity and the specificity of near best corrected visual acuity (BCVA) and normal dilated fundus examination (DFE) for macular anomalies detection before routine cataract surgery. To describe the type and the incidence of macular abnormalities pre-operatively found by Spectral-Domain Optical Coherence Tomography (SD-OCT).

Methods:
Design: Retrospective monocentric cohort.
Methods: All patients scheduled for cataract surgery between October 2016 and March 2017 were enrolled. Datas included before the surgery were near and far BCVA, type and stage of cataract, DFE and macular SD-OCT. Macular anomalies were suspected if near BCVA was worse than Jaeger 1 (J1) and/or if DFE was abnormal. The gold standard test was the macular SD-OCT scan line.
Results:
303 eyes were eligible and 297 eyes were included. One hundred and two (56.9%) patients were females. The mean age was 72.5 years (sd 8.8). Six SD-OCT scans (2%) were not available because of media opacities. Macular anomalies were found in 73 eyes (24.6%). The sensitivity of J1 BCVA and normal DFE was 76.2%, the specificity was 94.4%. The most common macular condition was age related macular degeneration (37 eyes, 12.4%) followed by idiopathic epi-retinal membrane (17 eyes, 5.7%). Less frequently, we observed 11 cystoid macular œdemas (3.7%), 3 vitreo-retinal tractions and lamellar macular holes (1.0% each).
Conclusions:
The sensitivity of near BCVA and DFE was moderate and appears to be inappropriate to detect small macular abnormalities. Preoperative macular SD-OCT scanning should be systematic before cataract surgery.
